What is Home Care?
Many times a loved one is stricken with an illness, injury or disability. When this occurs there is still no place like home. These changes can be managed at home with skilled professional health services provided by the staff of Heritage Home Health Services.
Our team of professionals can help you or your family member if they are convalescing from an illness or surgery for example:
Whether you want to talk to someone about your care or are trying to decide if home care is the right choice for you, we have a nurse on call 24 hours a day, seven days a week. We’re always there to help you feel right at home.
- health care problems pertaining to the digestive system
- heart disease
- drainage tubes
- ostomy care
- limited mobility
- infected wounds or burns
Am I Eligible for Home Care Services?
Your physician must certify that you need home care services.
Medicare and most Private Insurance require you to be homebound and need a skilled professional such as a nurse or therapist. Once you meet Medicare criteria it will also pay for a home Health Aide and Medical Social Services.
With a Prior Authorization from Medicaid they will pay for Skilled Nursing, Home Health Aides, and Therapists.
All services are available under Private Pay.
How Do I Get Home Health Care?
If you are at home, call your physician or our office at (765) 342-2126.
If you are in the hospital, contact the discharge planner, social worker or your physician.
If you are in a nursing home or a convalescent center, contact the social worker or talk to your physician.
Heritage Home Health Services is Medicare and Medicaid certified.
